Registration Information

Payments

All tournament payments are due prior to the accepted teams being posted. If your payment has not been received by this time your team may not be accepted. 

No refunds will be issued after Brackets have been posted.

All teams must check-in at least one hour before their first game or by noon Saturday morning (see #3 below). Check-in can be completed in one of 3 ways:

 

1.  Pre-check-in by mail – all forms must be received by 5/9/2016. Forms can be sent to:

NOTE: If you mail in your check-in information you will be notified if your information is incomplete.

You will still need to bring your player passes to registration for verification at #2 or #3 below.

 

2.  Pre-tournament check-in – this will be held on Friday 5/19/2017  from 5:00-9;00 pm at:

Holiday Inn
3202 East Empire St
Bloomington, IL 61704

 

3.  For those teams that have games later on Saturday we will have check-in available from 7am-11am at Community Fields Registration Tent next to the main building.

 

The following information outlines all of the forms required for this tournament. Please review this thoroughly and ensure that your team representative has all required items at registration.

 

 

1.  Official State Roster

This is the team’s official state roster signed by state officials.

 

2.  IYSA Tournament Roster

This tournament roster is required from all teams, whether they are in-state or out-of-state teams. It should list only those players playing in the tournament.

 

3.  Illinois Medical Release & Waiver:

a.  This form is required for each player that is on the IYSA Tournament Roster.

b.  For IYSA teams, The forms must be shown at check- in. We will not need to keep a copy but the forms are required to be at each game. In the event of an injury, a copy needs to be given to the trainers.

c.  All other teams must submit a copy of the Medical Waivers to the tournament to keep on file. So, bring extra copies as they will not be returned.

 

4.  Non-IYSA/US Team (Form NT) 

This form is required from all teams that are not IYSA, which includes US Club. please bring signed.  

 

5.  Travel Permit

This form is required for all Out of State teams and is the USYSA form approved by your state association.

 

6.  Guest Player Permit:

a.  For guest players from the same club we only require a copy of team’s roster that the guest player is officially on. the player must be age appropriate. No exceptions.

b.  For guest players outside the club an ISYA guest player permit will need to be submitted and signed by both loaning coach and coach of the team they will be playing on.

 

 

Along with the above forms, player passes must be presented at check-in for verification. Player passes will be banded at registration.

 

Thank you in advance for your help in making this process as easy on our volunteers as you can. Submitting the information in advance by mail will make your time at check-in very short and can help us ensure we notify you of any issues with your paperwork before you leave home.
