At Registration:

- 2007-2008 State Approved Roster with any add/drop forms (non US Club teams)
- Player Passes - USYSA or US Club
- Illinois Medical Release Form - every player must complete this form but it does NOT need to be notarized and copies of insurance cards are NOT needed
- IYSA Sanctioned Tournament Roster - required for ALL teams by IYSA
- Guest Player Permits - any guest player must have a permit from the teams appropriate state association (non US Club teams)
- Travel Permit - all non-Illinois teams must have a travel permit (non US Club teams)
- 2007-2008 Tournament Team Agreement will be required for US Club teams only, as well as all items listed on the Agreement. Read this document carefully. The additional items required by this document include, but are not limited to, a US Club
certificate of liability insurance and an IYSA disclosure statement for all adults associated with the team.
